I haven't heard of this issue affecting H811 or H815 models (the more popular models). They pulled the batch from the shelves and didn't sell any additional handsets until a fixed batch came in. Also, called LG and they were not helpful, as this particular phone is not in a boot loop. However, there is drop damage to two of the corners on the device. This one was manufactured in November, so it may not be affected. Interesting fact though: If you do three warranty returns with in 90 days, T-Mobile MUST offer you a new phone. There is no guarantee that this phone will be v1.1. The T-mobile rep spent sometime on the phone with customer care, and they are sending out another phone.

Update 4: Went to the T-mobile store, where they were unable to exchange/upgrade my phone. Pretty sure I got lucky by having a tech who had experienced the boot loop issue on her G4. Mentioned some minor funky behavior of the new phone, and they are sending me to the store for an exchange/upgrade to 1.1.

Was initially turned down for an upgrade to v1.1. Called the T-mobile store, where I had just returned my loaner phone. Update 2: T-mobile sending out replacement Tuesday, and 'gave' me a loner.
